Care Management Service
Care Management Service works alongside children, young people and families subject to child in need, child protection plans, legal proceedings (private and public law), looked after children, families who have no recourse to public funds and children with disabilities.
The Care Management Service is part of Sandwell Children's Trust
The Service is made up of team managers, experienced social workers, newly qualified social workers, student social workers, and social care assistants. They work in close partnership with children, young people and families as well as other key professional agencies such as schools, GPs, voluntary sector and probation so that the plans that are put together for the individual children and their families show outcomes and are making a difference to their lives.
Social workers work with individual children and young people who have different plans depending on their identified needs, for example, child in need plan, child protection plan, looked after children’s plan. These plans are usually managed through a series of structured home visits to undertake direct work with children, young people or parents/ carers as well as meetings and reviews to measure if change is being achieved and if children’s needs are being met.
In some cases where change is taking longer to achieve due to a number of reasons, a legal planning meeting is arranged to consider whether a follow Public Law Outline (PLO) or if the child is in immediate danger, the police can take a child into ‘police protection’ or make an application to court for an Emergency Protection Order (EPO).
Care Management Service also work in conjunction with different internal departments within the Trust, who support them in achieving the coordination of tasks necessary to safeguarding and protect children in Sandwell.
